New Clients, before your first appointment please do the following:

1. Click on this link and fill out the Online Client Intake Form, and the Federal HIPAA Disclosure Form.
2. Download, print and fill out the New Client Intake Packet and bring it with you to your first appointment.
3. The HIPAA form is for your information.  You can read the HIPAA form on your computer or print and read it as you prefer.  Federal HIPAA Disclosure Form
